RETRIEVE FDA PIN NUMBER
How To Retrieve Your FDA Facility Registration Pin Number: A Pin Number (also referred to as a "PIN code") is a combination of upper/lower case letters and numbers. If you do not know your facility’s Pin Number, the owner, operator, or agent in charge of your facility can request either Number from FDA by the following methods:
BY EMAIL Send an email to furls@fda.gov requesting the relevant information. The email should include the company’s name, address, and facility telephone number. FDA will verify that the individual requesting the Pin Number is the owner, operator, or agent in charge of the facility. Upon successful verification of the requester, the Pin Number will be sent to the owner, operator, or agent in charge of the facility by email, or mail as appropriate. IN WRITING Send a letter requesting the relevant information, on company letterhead, and include the company’s name, address, email address, and facility telephone number to:

BY FAX Send a fax to +1 (301) 436-2804 and request that the relevant information be provided. Be sure to write the fax on letterhead and include the company’s name, address, email address, telephone number, and reply fax number.
Note: Once retrieved, please relay your Pin Number to registration@unitedsafetyagents.com. It is a requirement when transferring the management of a facility's FDA Registration.
